Nurse Practitioner is a clinically competent professional provider who manage, prescribes, delegates, and coordinates care. This practitioner provides care at patient’s houses and also provide care through different telehealth platforms such as: telephone, televisit, telemonitoring and chat among others. Other duties includes but are not limited to: orientation and education to patients. Effective and efficient interactions with patient’s families, caregivers, and other members of the health team.


Essential Job Functions
  1. Review clinical records and perform physical examinations, by evaluating the patient's medical condition and health status, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documenting the findings in the patient's medical records.
  2. Support management of chronic and acute conditions, as well as exacerbations.
  3. Prescribe medications and other therapeutic alternatives as appropriate, and observe patient response to care plan and clinical evolution.
  4. Refers patients to specialists and to relevant patient care components as appropriate
  5. Directs and coordinates the patient care activities and support.
  6. Follow established policies, procedures, guidelines and protocols set up by the company.
  7. Ensure the compliance of treatment plan in order to reach the goals and objectives. If needed, define an improvement plan agreed to with the patients.
  8. Comply with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state and organizational policies
  9. Ask for support and guidance from the Physician and Medical Director when patient's condition require
  10. Talk directly to patients on the telephone, chat and/or televist direct patients to the most appropriate service when needed.
  11. Work closely with the Care Coordinator and other team members to arrange home visit appointments for patients.
  12. Receives to in-coming telephone calls from patients requiring medical guidance. Provides patients with clear instructions and education.
  13. Directs patient to the suitable care including emergent care, urgent care, follow-up appointment.
  14. Promotes wellness and informed decisions by providing general health information, home care advice and/or prevention concepts.
  15. Communicates with physician or other health care providers regarding patient's status, in a concise manner to bring issues to an effective resolution.
  16. Responsible for receiving and responding to Telemonitoring alert system. Review of the patient electronic health record for early identification of protocol driven interventions
  17. Coordinates and collaborates with providers, the medical directors, nurses, social workers and other professionals to evaluate and address patient needs.
  18. Educating the patient and family members about medical conditions, managing of illness, courses of action, medications, side effects, treatment, illness progression, diet and nutrition, medical adherence.
  19. Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state, and organizational policies.
Required Education
  1. Current unrestricted Florida State Medical License with no current disciplinary action
  2. Current State D.E.A. Certificate.
  3. Degree from an accredited Nurse Practitioner Program
  4. BLS and ACLS valid certificate.
  5. Any combination of education, training and experience which demonstrates the ability to perform the duties and responsibilities as described, including related work experience.
Preferred Education and Experience     
  1. English proficiency is a must. Candidates with  abilities to speak other languages such as: Spanish, Creole or others are highly encouraged to apply.
  2. Relevant or any other job-related vocational coursework.
